It is important for patients to abide by all clinic policies below:
Medical Emergencies
Our clinic is not equipped to handle medical emergencies. If you have a medical emergency, please call 911 or go to the nearest emergency department.
Missed Appointments and Late Arrivals
When you book an appointment at our clinic, that time is specifically reserved for you. If you miss an appointment without providing 24-hour notice, you will be charged. Please check in on time, or you may need to reschedule. Repeated no-shows or late cancellations may lead to termination of the patient-physician relationship.
Waiting Times
We understand that waiting can be inconvenient. Our physicians often face unexpected emergencies and patients needing additional time. We appreciate your patience and assure you that you will receive the same dedicated care when it's your turn.
Health Insurance Coverage
To receive services covered by OHIP, please present a valid health card at each visit. If you do not have a valid health card, you will need to pay for the services.
Non-insured Services and Forms
Some services and procedures are not covered by OHIP and will require out-of-pocket payment. Fees are posted in the clinic and are in line with recommendations from the Ontario Medical Association. Physicians at the clinic will use their discretion when asked to fill out a form for a patient. Physicians are not able to fill out all forms given the episodic nature of the physician-patient relationship.
Opioid Prescriptions
Our physicians may prescribe opioids for cancer-related pain but might not prescribe them for chronic non-cancer pain. Patients receiving opioid prescriptions will need to complete an Opioid Agreement.
Communication of Results
Lab and imaging results typically are received between 2-4 weeks, depending on the type of exam. We require follow up appointments to be booked for communication of exam results. We are unable to discuss results over the phone. We will make a maximum 3 attempts to book a follow up appointment to discuss results. Please contact the clinic if you have not received your results in 4 weeks time.
Privacy Commitment
We are committed to protecting your privacy. We take measures to secure your personal health information from theft, loss, and unauthorized access. All staff members are required to protect your privacy and use your information only as allowed by law.
Collection of Personal Information
We collect personal health information under the authority of the Personal Health Information Protection Act, 2004. This information includes your name, date of birth, address, health history, and records of your visits to Greely Walk-In. Generally, we collect this information directly from you or your authorized representative.
Use of Personal Health Information
Access to your personal health information is limited to those who need it to provide care, including physicians and nurses. Additionally, your information may be used for education, research, statistical analysis, compliance with legal requirements, program management, and quality improvement activities.
Disclosure of Personal Health Information
Unless directed otherwise, we may share your health information with care providers and electronic health systems to support your care. We may also disclose it for legal, research, and public health purposes as permitted by law.
Policy on Patient Conduct
Inappropriate or abusive behaviour towards staff, physicians, or other patients is not tolerated. Such behaviour can lead to the termination of the patient-physician relationship and exclusion from the clinic. Please maintain respectful interactions at all times.
Feedback and Complaints
We value your feedback to improve our services. Discuss any concerns with your physician. For formal complaints, please submit them in writing to our Clinic Manager of Greely Walk-In to ensure appropriate follow-up.
